Dayton Center for Neurological Disorders is open Monday-Friday from 8am-4pm.

DCND’s main office is conveniently located on the Southview Hospital campus, with secondary locations and satellite offices throughout the Miami Valley. Our main office is located at 1975 Miamisburg-Centerville Rd. Centerville, OH 45459. For directions click here.

If you need to cancel or reschedule your appointment, please contact our office a minimum of 24
hours prior to your scheduled appointment time. Call 937‐439‐6186.

Arriving Late:  If you are more than 10 minutes late for your scheduled appointment time, we will
attempt to accommodate your needs. However, this is not a guarantee that the physician or PA can see
you. You may have to be rescheduled.

First Missed Appointment:  This is defined as a failure to notify us by phone call or secure message
through the patient portal within the 24 hour cancellation/reschedule window. Missing your first
appointment will result in being placed on the end of DCND’s “cancellation list.” Please note that this
may result in a 3‐month wait for an appointment slot. You may be charged $50 as reimbursement to
the practice for time and resources lost.

Second Missed Appointment:  Failure to notify our office (as defined above) for the second time may
result in the cancellation of all future appointments and being terminated from the practice. Please keep
in mind that the 2 missed appointments do not have to be consecutive. You may be charged $50 as
reimbursement to the practice for time and resources lost.

  • Current Insurance Card
  • Driver License or State ID
  • Current list of medication, including dosage and frequency
  • Any pertinent information about your medical history
  • Any recent imaging discs or x-rays
  • Your insurance co-payment

The physicians at DCND are contracted with most major insurance plans including Medicare, United Healthcare, Anthem and Aetna, and select Medicaid plans. Our office will submit a claim on your behalf to your insurance payor for services provided. We cannot guarantee payment for any claims submitted. Please consult your plan for the list of benefits it provides. Our office does not do third party billing, i.e. personal injury due to auto accident, etc.

The portal is the preferred method of contact, but if you are unable to utilize the portal, please call our Medical Records department Monday through Friday between the hours of 8:00am-4:00pm at 937-439-6186; press option 4. The Health Insurance Portability and Accountability Act of 1996 (HIPPA) allows a fee to be associated with processing medical records requests. However, there is no fee for medical records requests sent directly to a physician or health care facility for continuity of care.

DCND is currently involved in a number of research studies. Differing neurologic conditions are the focus of each study, so please call our research coordinator at 937-424-3033 if you are interested in obtaining further information.
