The portal can be used to pay your bill, view online statements, or ask a question of one of our billers.
The portal is the preferred method of contact, but if you are unable to utilize the portal, our billing office is available Monday through Friday between 8:00am-4:00pm. Please call 937-439-6186 and press option 5 for billing.
Please be sure to notify DCND of any changes to your insurance, address or phone number.
The physicians at DCND are contracted with most major insurance plans including Medicare, United Healthcare, Anthem and Aetna, and select Medicaid plans. Our office will submit a claim on your behalf to your insurance payor for services provided. We cannot guarantee payment for any claims submitted. Please consult your plan for the list of benefits it provides. Our office does not do third party billing, i.e. personal injury due to auto accident, etc.
Office co-payments are due at the time of service unless prior arrangements are made. Our office accepts Cash, MasterCard, Visa and Discover.
Payments can be made in person, mailed, made over the phone with our billing personnel, or paid through our SECURE PATIENT PORTAL.
Paperwork Processing for Disability Related Forms
Please allow 7-10 business days from the date we receive for processing. We do not process forms the same day. Please make sure that all patient sections are filled out completely and a current “records release form” is on file. There is a fee associated with most forms which is due prior to the completion of the form. For any questions regarding these fees contact our billing department.
