The Dayton Center for Neurological Disorders (DCND) proudly partners with Craniometrix to offer Medicare's Guiding an Improved Dementia Experience (GUIDE) Program for qualifying patients. This free support program provides personalized care coordination, caregiver education, respite services, and ongoing support to help patients and their caregivers navigate dementia with greater confidence. If you think you or a loved one may qualify, ask your primary care provider or neurologist for a referral. You can then contact our team by phone or online to schedule your onboarding consultation.

What is the GUIDE Program?

The Guiding an Improved Dementia Experience (GUIDE) Program is a Medicare initiative that helps improve the quality of life for people living with dementia while providing support for their caregivers. Eligible Medicare beneficiaries receive personalized care coordination, individualized care plans, caregiver education, and connections to community resources.

At DCND, we partner with Craniometrix to provide GUIDE Program services while continuing your specialized neurological care. Together, we help patients stay as healthy, safe, and independent as possible while supporting caregivers every step of the way.

  4. Existing patients who qualify will be contacted by Craniometrix
